The Administration of Medication form has sections that must be completed and signed by the parent, the building principal and a licensed medical provider. The form must be on file at your child's school before any medications will be administered.
The completed Administration of Medication form is required for all prescription medications, inhalers, epi-pens, insulin and over-the-counter medications.
If you have questions or concerns regarding the administration of medications while a student is in school, please contact your school nurse.
